A shampoo showed up on my doorstep a couple weeks ago. Head & Shoulders Menthol Fresh, 400ml, the one with the blue and white bottle that’s been sitting in supermarket aisles since basically forever. Sent to me for review. And my first thought, if I’m honest, was “great, another dandruff shampoo that’s going to do absolutely nothing for actual seborrhoeic dermatitis and is just going to sit under my sink.”
I was wrong. Or at least, wrong enough that I want to walk you through it properly.
So here’s what I did. I pulled it into my regular seb derm rotation, swapped it in for one of my usual shampoos, and used it for over a week straight to see what it actually did on a scalp that’s dealing with something a bit more stubborn than run-of-the-mill flakes.
Quick disclosure before I get into it: this was sent to me for review. I’m not being paid to say anything nice about it, and everything below is what actually happened when I used it.
First, the smell and that cooling thing
It smells clean. Light, fresh, a bit minty without smacking you in the face with it. Nothing like those dandruff shampoos from the 90s that smelled medicinal enough to make you feel like you were being punished for having a scalp condition. This one just smells like a normal, pleasant shampoo that happens to also be doing a job.
And then there’s the cooling sensation. It shows up the second the shampoo hits your scalp, this tingly, minty coolness, and here’s the bit that actually surprised me: it doesn’t stop when you rinse. I could still feel it faintly after I’d towel dried and moved on with my morning. That’s the menthol doing its thing, and I know for some people that’s the whole appeal. For me, it’s a little more than I’d personally choose. Not a dealbreaker. Just not my favourite sensation to still be feeling twenty minutes after I’ve left the shower.
Ingredients worth knowing about (and the seb derm safety check)
Right, this is the bit I actually wanted to dig into, because “is this seb derm safe” isn’t really a yes or no question, it’s an ingredients question. I ran the current UK formula through my usual checks the same way I do for everything in my Seb Derm Directory, and here’s what’s actually going on.
Key active ingredient at a glance

Piroctone Olamine
The active ingredient is Piroctone Olamine, not Zinc Pyrithione. Worth flagging because if you go looking online, you’ll find older listings for “Menthol Fresh” (usually the 500ml bottles) that still list Zinc Pyrithione as the active. The formula’s clearly been updated. The current 400ml stock, which is what I’ve got and what matches the official Head & Shoulders UK product page, runs on Piroctone Olamine under what they’re calling their “Microbiome Balance” formula.
That actually matters for the seb derm question, because Piroctone Olamine isn’t just a marketing ingredient. It’s a genuine antifungal that targets Malassezia, the yeast that’s the actual root cause of both dandruff and seb derm flares. It doesn’t come with the skin-thinning risk you get with steroid treatments, or the harshness some people get from stronger antifungals like ketoconazole. It’s a proper active doing a proper job, not just a cosmetic word on a bottle.
Sodium Salicylate
A nice bonus in there. It’s a mild keratolytic, basically a gentler cousin of salicylic acid, and it helps with breaking down flaky buildup on the scalp surface.
What’s not in there
This is the bit I was specifically checking for. No coconut oil, no plant butters, nothing sitting high up the ingredient list that’s going to actively feed the yeast you’re trying to control. There is Cocamide MEA and Glycol Distearate in there, both pretty standard in most shampoo formulas including a lot of the ones I already recommend, and generally low risk given where they sit in the list, but as always, if you’re very reactive I’d run the current INCI through Sezia yourself before committing, because formulas do change.
The fragrance flag
The one thing I’d flag for anyone with fragrance sensitivity alongside their seb derm: there’s Parfum in there, plus Linalool and Hexyl Cinnamal, which are fragrance components that can be a problem for people with reactive skin generally. That’s not a seb derm issue specifically, it’s a “does your skin get annoyed by scented things” issue, but it’s worth knowing before you buy.
So, is it seb derm safe? Based on what’s actually in the bottle, yes, and the active ingredient gives it a genuine reason to help rather than just cosmetically cover things up. It’s not going to replace a ketoconazole shampoo during a proper flare (nothing on the piroctone olamine tier really does), but as a rotation shampoo for keeping things calm day to day, the formula holds up.

How it actually performed
Okay, the bit you’re actually here for.
I used this for over a week, in place of one of my usual rotation shampoos, specifically to see how my seb derm scalp reacted. No flare ups. My scalp stayed calm and under control the entire time, which for anyone who’s dealt with seb derm knows is genuinely not nothing. A week is enough time for a bad ingredient to show its hand, and this one didn’t.
It cleanses properly too. Not stripped-feeling, not that squeaky “I’ve just washed all the natural oils off my head” sensation some anti-dandruff shampoos give you. Just clean, and my hair felt normal afterward rather than dry or coated.
Would I say it did more than maintain? Honestly, I’m not sure I can claim that off one week. What I can say is it held the line. My scalp didn’t get worse. Didn’t flare. Just stayed comfortable, which with seb derm is honestly the whole ballgame. For a mid-tier rotation shampoo, that’s exactly the job I need it to do.

Buy it if:
- You’re managing seb derm or persistent dandruff and want a rotation shampoo with a genuine antifungal active, not just a fragrance-forward “anti-dandruff” label
- You like a noticeable, invigorating cooling sensation on the scalp
- You want something widely available and not expensive
- You’re looking for something to sit alongside a stronger treatment, like a ketoconazole shampoo, rather than replace it
Skip it if:
- You’re sensitive to fragrance ingredients, since Parfum, Linalool and Hexyl Cinnamal are all present
- You don’t like the cooling, tingly feeling lingering after you rinse
- You’re mid-flare and need something stronger. This is a maintenance shampoo, not a flare-buster
My rating: 4.5/5. Genuinely good. My scalp didn’t flare once in over a week, and that’s down to an active ingredient that’s actually doing something, not just a nice-sounding label on a supermarket bottle. Half a point off, and it’s entirely personal: I’m just not that into how long the cooling hangs around after I rinse. If you love that tingly feeling, this might be a full 5 for you.
